1. Log into your Sport 80 personal account
2. Click on "View Profile"
3. Click on tab called "Complete Player Profile + Requirements"
4. Click blue button in top right to add form
5. Complete each tab and answer eligibility question (those that select "No" will be required to complete an eligibility waiver.)
6. Click "Add" in bottom right
